This study reports the first two cases of primary sinonasal choriocarcinoma, a rare germ cell tumor. Early identification and specific therapy are crucial for managing this aggressive sinonasal neoplasm.

Background:

  • Primary choriocarcinoma of the sinonasal tract is exceptionally rare and has not been previously documented.
  • Germ cell tumors typically occur in reproductive organs, but can arise in extragonadal sites.

Observation:

  • Two male patients presented with epistaxis and nasal obstruction.
  • Imaging revealed sinonasal masses, confirmed histopathologically as choriocarcinoma with cytotrophoblastic and syncytiotrophoblastic cells.
  • Tumors showed strong keratin and beta-chorionic gonadotrophin (HCG) positivity, with elevated serum HCG levels.

Findings:

  • The study documents the first two cases of primary sinonasal choriocarcinoma.
  • Histopathology confirmed a dual cell population characteristic of choriocarcinoma.
  • Immunohistochemistry and elevated serum HCG levels supported the diagnosis.

Implications:

  • These cases support the existence of primary choriocarcinoma in the sinonasal tract, expanding the known distribution of germ cell tumors.
