Summary

Reoperative central compartment dissection for thyroid cancer recurrence has low morbidity. Permanent recurrent laryngeal nerve paralysis occurred in 5.2% and permanent hypoparathyroidism in 9% of patients when the nerve was not invaded.

Area of Science:

  • Endocrinology
  • Head and Neck Surgery
  • Oncology

Context:

  • Reoperation for recurrent thyroid carcinoma presents unique surgical challenges.
  • Central neck lymph node dissection (level VI) is crucial for staging and treatment of recurrent disease.
  • Previous surgery can alter anatomy, increasing risks.

Purpose:

  • To evaluate the incidence of permanent recurrent laryngeal nerve paralysis and hypoparathyroidism following reoperative central compartment dissection (level VI).
  • To assess the safety and morbidity associated with re-dissection of the central neck compartment in patients with recurrent thyroid cancer.

Summary:

  • A retrospective study analyzed 18 patients undergoing 22 reoperative central compartment dissections for recurrent thyroid carcinoma or metastatic lymph nodes.

  • Permanent hypoparathyroidism occurred in 2 patients (9%) and permanent recurrent laryngeal nerve paralysis in 4 patients (5.2%).
  • Nerve paralysis was intentional in 3 cases for oncologic reasons; the fourth occurred during a complex second reoperation.
  • Impact:

    • Central neck reoperation can be performed with acceptable morbidity, particularly when the recurrent laryngeal nerve is preserved.
    • Highlights the importance of careful identification and preservation of the inferior laryngeal nerve during reoperative central compartment dissection.
    • Provides data to inform surgical decision-making and patient counseling regarding risks of re-operation for recurrent thyroid cancer.
