Background:

Observation:

  • A patient with known carmine hypersensitivity presented with anaphylaxis post-ingestion of generic azithromycin.
  • Skin-prick testing revealed hypersensitivity to carmine dye and the coating of the generic azithromycin tablet, but not the active antibiotic or the brand-name formulation.
  • The brand-name azithromycin (Zithromax) lacked carmine dye and was tolerated.

Findings:

  • The patient's anaphylaxis was attributed to carmine dye in the generic azithromycin tablet coating.
  • Skin-prick tests confirmed allergy to carmine dye and the specific colored coating.
  • This is the first reported case of carmine-induced anaphylaxis linked to a carmine-containing medication.

Implications:

  • Accurate patient history and targeted allergy testing are vital for diagnosing medication hypersensitivity.
  • Patients with carmine allergy must verify medication formulations to avoid adverse reactions.
  • Awareness of dye-related allergies can prevent unnecessary avoidance of essential antibiotic classes.
