    Summary

This case study highlights challenges in orthodontic treatment for patients with significant bone loss. Utilizing temporary crowns on osseointegrated fixtures aided final orthodontic movements, suggesting earlier placement could streamline treatment.

    Area of Science:

    • Dentistry
    • Orthodontics
    • Prosthodontics

    Background:

    • The patient presented with significant bone loss in the anterior edentulous region (21-23), necessitating a removable prosthesis for missing teeth.
    • A Class II Division 1 malocclusion required orthodontic intervention to address maxillary proalveoli and align mandibular incisors.

    Observation:

    • The extensive anterior edentulous area complicated orthodontic treatment, precluding continuous multiring mechanics and requiring sequential removable prostheses for anchorage.
    • Temporary crowns on osseointegrated fixtures (replacing teeth 22 and 23) were employed to achieve necessary final orthodontic movements for preprosthetic treatment.

    Findings:

    • The strategic placement of temporary crowns on osseointegrated fixtures facilitated the completion of complex orthodontic movements.
    • Earlier placement of osseointegrated fixtures could have accelerated and simplified the overall orthodontic treatment process.

    Implications:

    • Integrating osseointegrated fixtures earlier in treatment planning can enhance efficiency and predictability in complex orthodontic cases.
    • Advanced technologies like 3D scanning, surgical guides, and specialized prostheses could overcome challenges in ideal fixture placement for combined orthodontic and prosthetic goals.
