Summary

Background:

  • Conventional laparoscopic instruments lack the degrees of freedom (DOF) required for complex surgical tasks in confined spaces.
  • Newer articulating laparoscopic instruments provide enhanced flexibility but may present control challenges due to increased complexity.

Purpose of the Study:

  • To compare task performance between articulating and conventional laparoscopic instruments.
  • To evaluate the learning curve and skill transfer for articulating instruments in both expert and novice surgical groups.

Main Methods:

  • Eight experienced laparoscopic surgeons (experts) and eight individuals with no surgical experience (novices) performed standardized tasks (peg transfer, suturing) using both instrument types.
  • Performance was assessed using a 100-point scale measuring speed and accuracy.
  • Participants were retested with articulating instruments after a brief orientation session.

Main Results:

  • Experts consistently outperformed novices (p < 0.001).
  • Overall, conventional instruments showed slightly better performance than articulating instruments (p = 0.074) in initial trials.
  • Novices demonstrated a significantly greater percentage improvement (27%) with articulating instruments after orientation compared to experts (1%).

Conclusions:

  • Experienced surgeons can effectively transfer their skills to articulating laparoscopic instruments.
  • Articulating instruments can be introduced early in surgical training to accelerate the learning process and improve proficiency.
