  • Radionecrosis of soft tissue and bone presents a significant reconstructive challenge.

Observation:

  • A 58-year-old patient developed severe chest wall radionecrosis with infection after mastectomy and radiation.
  • An initial TRAM flap reconstruction failed due to ischemia and necrosis.
  • Vacuum-assisted closure (VAC) therapy was used to prepare the wound bed.

Findings:

  • A pedicled omental flap combined with Integra(R) artificial skin and a split-skin graft achieved complete wound closure.
  • This Integra-omental flap technique effectively reconstructed the extensive chest wall defect.

Implications:

  • The Integra-omental flap offers a viable solution for complex chest wall reconstruction post-radiation.
  • This approach may improve outcomes for patients with extensive tissue loss and infection.
