Background:

  • Inflammatory pseudotumors are rare benign lesions that can occur in various organs.
  • Metachronous presentation, where tumors arise at different times in different locations, is uncommon.

Observation:

  • A 78-year-old male patient with a history of orbital inflammatory pseudotumor developed a new lung mass.
  • Elevated immunoglobulin G (IgG) and antinuclear antigen levels were detected serologically.

Findings:

  • Histological examination revealed lymphoplasmacytic infiltration and dense fibrosis.
  • Flow cytometry confirmed the polyclonal nature of the infiltrating cells.
  • Immunostaining identified numerous IgG4-positive plasma cells, indicating an IgG4-related process.

Implications:

  • This case expands the understanding of IgG4-related disease presentation.
  • Highlights the importance of considering systemic involvement in patients with inflammatory pseudotumors.
  • Suggests potential for novel diagnostic and therapeutic strategies for IgG4-related orbital and pulmonary lesions.
