Background:

Ileal pouch-anal anastomosis (IPAA) is a critical surgical procedure.
Comparing laparoscopic and open approaches is essential for optimizing patient outcomes.

Purpose of the Study:

To compare safety, early and late outcomes, quality of life, and functional results of laparoscopically assisted versus open IPAA.

Main Methods:

Retrospective matched-cohort study of patients undergoing IPAA between 1992 and 2007.
Matched patients (1:2 ratio) based on age, sex, BMI, and operation date.
Quality of life assessed using the Cleveland Global Quality of Life scale at 1 and 5 years.

Main Results:

Similar 30-day and long-term outcomes between laparoscopic and open IPAA groups.
Laparoscopic IPAA demonstrated a shorter median time to stoma action (2 vs. 3 days) and marginally shorter hospital stay.
Longer median operating times were observed in the laparoscopic group (272 vs. 163 minutes).

Conclusions:

Laparoscopically assisted IPAA is a safe and effective alternative to open IPAA.
The laparoscopic approach offers short-term functional advantages without compromising long-term results or quality of life.
