Summary

HIV infection significantly increases tetanus mortality risk, especially when there is no identifiable entry point. This highlights the need to revise tetanus classification systems to account for HIV status.

Background:

  • Tetanus remains a significant public health concern, particularly in regions with lower vaccination rates.
  • The interplay between Human Immunodeficiency Virus (HIV) infection and tetanus outcomes requires further investigation.

Purpose of the Study:

  • To elucidate the relationship between HIV infection and the clinical presentation and outcomes of tetanus.
  • To assess the prognostic value of HIV status and portal of entry in tetanus patients.

Main Methods:

  • A prospective study was conducted on patients admitted with tetanus between July 15 and December 31, 2007.
  • Screening for HIV-1 and 2 was performed on all included patients.

Main Results:

  • Of 21 tetanus patients, 57% were co-infected with HIV, with a mean CD4 count of 157/mm3.
  • Overall mortality was 53%, significantly higher in HIV-positive patients (82%) compared to HIV-negative patients (18%).
  • Absence of a portal of entry and HIV co-infection were identified as poor prognostic factors.

Conclusions:

  • HIV infection and lack of a discernible portal of entry are critical indicators of poor prognosis in tetanus.
  • The Dakar International Classification for tetanus should be revised to incorporate HIV status and absence of portal of entry as severity markers.
