Young adults with ischemic stroke show improved outcomes with thrombolytic treatment. Intravenous alteplase demonstrated good safety and efficacy in this patient group.

- Mortality was 0% in young alteplase-treated patients during the 3-month follow-up.

- Intravenous thrombolysis with alteplase is safe and effective for young adults with acute hemispheric ischemic stroke.
- Younger patients appear to benefit more from thrombolysis compared to older counterparts.
- Thrombolytic therapy should be considered in the management of ischemic stroke in young adults.
