Diverticular Disease of the Colon01:27

Diverticular Disease of the Colon

Diverticular disease involves the formation of diverticula—small sac-like outpouchings of the colonic wall—and their complications. It most commonly affects the sigmoid colon due to higher intraluminal pressure and structural vulnerability. It results from structural weakness and increased pressure in the colon, producing pseudodiverticula that may remain silent or progress to inflammation and serious complications.Structure of DiverticulaIn diverticulosis, these outpouchings are...

Inflammatory Bowel Disease V: Surgical Management

Surgical interventions for inflammatory bowel disease (IBD), which includes ulcerative colitis and Crohn's disease, are essential in managing symptoms and addressing complications. The selection of surgical procedures is contingent upon the specific conditions and complications that stem from these illnesses.
Here are some common surgical interventions for IBD:
Appendicitis01:19

Appendicitis

Appendicitis is an acute inflammatory condition of the vermiform appendix, most commonly caused by obstruction of its lumen. The appendix is a narrow, blind-ended pouch that extends from the cecum, making it particularly prone to obstruction. Causes include fecaliths, lymphoid hyperplasia (often after viral infections), parasites, tumors, or foreign bodies. This obstruction initiates a cascade of pathological changes.Luminal Obstruction and Early InflammationAfter obstruction, normal mucosal...

Appendicitis-II: Diagnostic Studies and Management

Diagnosing and managing appendicitis requires a structured and comprehensive approach that spans from initial assessment to postoperative care. Here is an overview of the process:
Diagnosing Appendicitis
It requires a multifaceted approach, starting with a detailed physical examination to pinpoint the location and nature of the pain and identify any associated symptoms. Laboratory tests play a crucial role. A complete Blood Count (CBC) typically reveals leukocytosis (an increased number of...
Inflammatory Bowel Disease I: Introduction01:26

Inflammatory Bowel Disease I: Introduction

Inflammatory bowel disease is a group of chronic disorders marked by recurrent inflammation of the gastrointestinal tract due to an abnormal immune response against gut microflora. This leads to tissue damage. The two main forms are Crohn’s disease and ulcerative colitis.Crohn’s DiseaseCrohn’s disease is a relapsing inflammatory disorder that can affect any part of the GI tract, from the mouth to the anus. It involves all layers of the bowel wall (transmural) and shows “skip lesions” in which...
Chronic Bowel Disorders: Introduction01:17

Chronic Bowel Disorders: Introduction

Chronic bowel diseases are a group of long-term conditions affecting the digestive tract, characterized by inflammation and damage to the gut lining. These conditions primarily include irritable bowel syndrome and inflammatory bowel disease.
Irritable Bowel Syndrome (IBS) is a common disorder affecting the gastrointestinal tract. The distinctive feature is recurrent abdominal pain associated with altered bowel movements, manifesting as constipation, diarrhea, or fluctuating between both. The...

Summary

Complicated diverticular disease, often involving the sigmoid colon, requires prompt surgical intervention when conservative measures fail. Perforation is a common complication, and CT-scans aid diagnosis, though mortality remains a concern.

Area of Science:

  • Surgery
  • Gastroenterology
  • Clinical Medicine

Background:

  • Diverticular disease is increasingly prevalent in clinical practice.
  • Complicated diverticular disease presents significant challenges in surgical emergency units.

Purpose of the Study:

  • To retrospectively evaluate the experience with complicated diverticular disease.
  • To analyze complications, diagnostic methods, and treatment outcomes.

Main Methods:

  • Retrospective analysis of 46 cases of complicated diverticular disease (2004-2007).
  • Data collection on patient demographics, complications, diagnostic procedures (CT-scan), and surgical interventions.
  • Evaluation of outcomes including mortality rates.

Main Results:

  • A male preponderance (M:F 1:1.4) with a mean age of 62.9 years.
  • Perforation was the most common complication, predominantly affecting the sigmoid colon (71.7%).
  • CT-scan was the primary diagnostic tool; laparoscopic surgery was used in 3 cases. Hartmann procedure was performed in 15.2% of patients. Global mortality was 8.1%.

Conclusions:

  • Complicated diverticular disease necessitates urgent surgical treatment when conservative approaches fail.
  • CT-scan is a valuable diagnostic tool, despite limitations.
  • The Hartmann procedure remains a frequently employed surgical option.
