Background:

Natural Orifice Transluminal Endoscopic Surgery (NOTES) aims to reduce abdominal incisions.
Laparoscopic surgery is standard for cholecystectomy but involves abdominal incisions.
Hybrid techniques seek to combine the benefits of different minimally invasive approaches.

Observation:

A novel hybrid technique integrating needloscopy with transvaginal NOTES was developed.
Two female patients with symptomatic cholelithiasis underwent successful needloscopy-assisted transvaginal cholecystectomies.
The procedure replicated standard laparoscopic cholecystectomy steps but via a transvaginal endoscopic approach.

Findings:

The hybrid technique resulted in less abdominal trauma compared to standard laparoscopy.

Gallbladder removal and clipping maneuvers were successfully performed through the transvaginal route.

The approach demonstrated feasibility for treating symptomatic cholelithiasis.

Implications:

Hybrid techniques represent a valuable intermediate step towards achieving pure NOTES.
