Ovaries

The ovaries are roughly the size of almonds and measure approximately 2 to 3 centimeters in length. These paired structures are situated within the pelvic region and are anchored by the mesovarium—a peritoneal extension that also connects them to the wider structure of the broad ligament. Oogenesis

Oogenesis,  the process of developing egg cells (female gametes), occurs within the ovaries and is fundamental to female fertility. This sequence begins during fetal development when diploid oogonia in the developing ovaries undergo mitotic divisions to produce primary oocytes. By birth, these primary oocytes enter prophase I of meiosis but become arrested in this stage, remaining suspended until puberty.

Background:

  • Primary osteosarcoma of the ovary is an extremely rare and highly malignant tumor.
  • Few cases have been documented in medical literature over recent decades.

Observation:

  • A 50-year-old postmenopausal woman presented with a large abdominal mass, initially diagnosed as ovarian cancer.
  • The patient's disease was aggressive, showing no response to chemotherapy and resulting in death within one month.

Findings:

  • Histopathological examination of autopsy specimens confirmed a diagnosis of primary ovarian osteosarcoma.
  • This rare ovarian tumor presents as a rapidly progressing pelvic mass.

Implications:

  • Primary ovarian osteosarcoma must be considered in the differential diagnosis of large, rapidly progressing pelvic masses in postmenopausal women.
  • Complete surgical resection is the only currently promising treatment, underscoring the importance of early diagnosis.
