Video-assisted extracorporeal appendectomy (VAECA) offers a shorter operating time and fewer complications compared to open appendectomy (OA). This minimally invasive approach results in better cosmetic outcomes and reduced hospital stays for appendicitis patients.

Background:

  • Laparoscopic appendectomy (LA) encompasses intracorporeal appendectomy (ICA) and video-assisted extracorporeal appendectomy (VAECA).
  • Open appendectomy (OA) remains a standard treatment for acute appendicitis.

Purpose of the Study:

  • To compare the feasibility and effectiveness of VAECA against OA.
  • To evaluate outcomes including operative time, complications, and hospital stay.

Main Methods:

  • A comparative descriptive study included 283 patients with acute appendicitis.
  • Patients were divided into two groups: OA (150 patients) and VAECA (133 patients).
  • Exclusion criteria included patients under 10 years and those with suspected appendicular mass.

Main Results:

  • VAECA demonstrated significantly shorter mean operating times compared to OA (p < 0.001).
  • VAECA group showed lower rates of operative complications (p < 0.001), postoperative complications (p < 0.01), and hospital stay (p < 0.001).
  • Postoperative infection incidence was higher in the OA group. Excellent cosmetic results were observed with VAECA.

Conclusions:

  • VAECA is a viable alternative to OA for simple appendicitis cases.
  • VAECA offers advantages in terms of reduced complications and improved cosmesis.
