Summary

Chorea gravidarum, a rare pregnancy complication, causes involuntary movements and speech changes. Symptoms typically resolve postpartum, but may recur in future pregnancies.

Background:

  • Chorea gravidarum is an uncommon pregnancy complication affecting 1 in 2275 pregnancies.
  • Characterized by involuntary movements, speech alterations, and affective status changes, typically in the first trimester.
  • Often idiopathic, but can be associated with antiphospholipid syndrome, rheumatic fever, thyrotoxicosis, systemic lupus erythematosus, syphilis, Huntington disease, or drug induction.

Observation:

  • A 22-year-old patient in her second pregnancy presented with psychosis and involuntary movements at 36.1 weeks gestation.
  • Initial treatment with diazepam was followed by amniotic rupture and subsequent Cesarean section.
  • The patient experienced a favorable postpartum recovery, with residual abnormal movement in her right hand.
