Summary

Single-modality radiation therapy (RT) provides excellent local control for imaging-defined meningiomas, comparable to surgery. This treatment offers a viable, low-morbidity alternative for select patients.

Background:

  • Meningiomas are primary tumors of the meninges.
  • Imaging-defined meningiomas lack histologic confirmation.
  • Treatment decisions often involve surgery or radiation therapy.

Purpose of the Study:

  • To evaluate the efficacy and toxicity of single-modality radiation therapy (RT) for imaging-defined meningiomas.
  • To compare RT outcomes with surgical resection benchmarks.

Main Methods:

  • Retrospective review of 41 patients with 42 imaging-defined meningiomas treated with RT alone (1985-2003).
  • Treatment modalities included stereotactic radiosurgery (SRS), fractionated stereotactic radiotherapy (FSR), and three-dimensional conformal therapy (3DCRT).
  • Median follow-up was 60 months; analysis used Kaplan-Meier methods.

Main Results:

  • High local control rate: 39 of 42 meningiomas (94% at 8 years).
  • Low toxicity: 1 patient experienced temporary neurologic sequela; no fatal complications.
  • Excellent 8-year actuarial freedom from complication survival rate of 97%.

Conclusions:

  • Single-modality RT is an effective treatment for imaging-defined meningiomas without significant mass effect.
  • RT offers local control comparable to surgery with significantly reduced morbidity.
  • Radiation therapy is a recommended alternative to surgery for various meningioma locations.
