This study introduces a new surgical protocol using cylindric osteotomes for tapered implants in type 4 bone. This technique enhances primary stability, enabling predictable immediate loading even in challenging bone types.

Observation:

  • A novel surgical protocol was developed for placing tapered implants in type 4 bone with wide ridges (> or = 8 mm).
  • The protocol exclusively uses cylindric osteotomes for alveolus preparation, avoiding conic osteotomes or drills.
  • The final cylindric osteotome size matches the conventional twist drill diameter.

Findings:

  • This technique optimizes bone compaction in the coronal third of the implant.
  • Improved bone density and enhanced primary stability values (> or = 70 ISO) were achieved.
