Summary
Intraorbital ophthalmic aneurysms are rare and typically do not rupture. Asymptomatic, unruptured aneurysms, like those found in this case, do not require treatment.

Background:
- A 52-year-old male presented with subarachnoid hemorrhage and multiple cerebral aneurysms.
- The patient exhibited Hunt-Hess grade 1 symptoms, including neck pain, nausea, and vomiting.
Observation:
- Cerebral angiography revealed multiple aneurysms: right posterior communicating, right anterior choroidal, left pericallosal, and bilateral intraorbital ophthalmic.
- Endovascular embolization was performed for all aneurysms except the intraorbital ophthalmic ones.
Findings:
- The intraorbital ophthalmic aneurysms were asymptomatic and therefore not treated.
- The patient was discharged without any neurological deficits following treatment of other aneurysms.
Implications:
- Intraorbital ophthalmic aneurysms are exceptionally rare with a very low risk of rupture.
- Conservative management is recommended for unruptured, asymptomatic intraorbital ophthalmic aneurysms.
- Surgical intervention is reserved for cases of rupture or significant mass effect.
