This study found that an educational intervention significantly improved medication error identification and reporting in hospice care. Hospice staff showed increased awareness and knowledge of medication errors after the training.
Area of Science:
- Palliative Care
- Medication Safety
- Healthcare Quality Improvement
Background:
- Medication errors pose a significant risk in hospice care, impacting patient safety and quality of life.
- Understanding the incidence and types of medication errors is crucial for developing effective interventions.
Purpose of the Study:
- To determine the incidence and characterize the type/severity of medication errors in hospice settings.
- To evaluate the impact of an educational intervention on medication error processes and reporting within hospice organizations.
Main Methods:
- Data on medication errors were collected and coded for type and outcome severity from two hospice organizations.
- An educational intervention comprising a 2-hour in-service and regular reminders was implemented.
- Participant knowledge and attitudes regarding medication error reporting were assessed via surveys at three time points.
Main Results:
- Medication error reporting increased in one hospice program following the intervention.
- Participant ability to correctly identify medication errors significantly improved (P < .001).
- Awareness of medication errors in hospice care increased significantly after the intervention (P < .01).
Conclusions:
- Educational interventions can effectively enhance medication error identification and reporting in hospice settings.
- Improved knowledge and attitudes among hospice staff contribute to better medication safety practices.
- Targeted education is vital for mitigating medication errors and improving care quality in palliative settings.
