Summary

Diagnosing microscopic virilizing ovarian neoplasms can be challenging. Ovarian vein sampling for androgens proved crucial in identifying a pure lipoid cell tumor as the source of excess testosterone.

Background:

  • Virilizing ovarian neoplasms are rare and can present with significant clinical manifestations of hyperandrogenism.
  • Accurate diagnosis is essential for appropriate management and treatment of these conditions.

Observation:

  • A woman presented with symptoms of severe hyperandrogenism, including hirsutism, voice deepening, and clitoromegaly.
  • Preoperative serum testosterone was markedly elevated (2,042 ng/dL), with extremely high levels detected in left ovarian venous blood (56,327 ng/dL).

Findings:

  • Despite initial negative findings, re-examination of ovarian tissue revealed a pure lipoid cell tumor.
  • Post-surgical bilateral salpingo-oophorectomy normalized serum testosterone levels to 20.7 ng/dL.
