Area of Science:

  • Endocrinology
  • Thoracic Surgery
  • Oncology

Context:

  • Retrosternal goiters represent a significant subset of surgical thyroid disease.
  • Understanding their demographics and clinical presentation is crucial for surgical planning.

Purpose:

  • To analyze the clinical characteristics, diagnosis, and surgical outcomes of retrosternal goiters.
  • To evaluate the safety and efficacy of the cervical surgical approach for retrosternal goiter removal.

Summary:

  • This study reviewed 363 goiter surgeries, identifying 60 (16.5%) retrosternal goiters.
  • Predominantly affecting females over 40, these were often multinodular, long-standing, and symptomatic.
  • Cancer incidence was 7%, and thyrotoxicosis occurred in 8% of cases.
  • Diagnosis relied on physical exam, chest X-ray, and scintigraphy; CT can be supplementary.
  • The cervical approach was used in 98% of cases with no operative mortality or major complications.

Impact:

  • Highlights the high symptomatic burden and surgical safety of retrosternal goiters.
  • Reinforces the cervical approach as a safe and effective surgical strategy.
  • Provides data on associated risks like cancer and thyrotoxicosis in retrosternal goiters.
