Summary

This case study highlights that keratoconus, a corneal disease, can occur in individuals with high hyperopia. Management with rigid gas-permeable contact lenses is a viable option for hyperopic keratoconus.

  • A 32-year-old male with a 10-year history of hydrogel contact lens wear presented with severe hyperopia.
  • Corneal topography (Orbscan) revealed bilateral asymmetric keratoconus, characterized by irregular astigmatism and abnormal corneal elevations.
  • Slitlamp biomicroscopy showed Vogt striae in the right eye.

Findings:

  • The patient exhibited high hyperopia with significant refractive error in both eyes.
  • Orbscan topography confirmed bilateral keratoconus with asymmetric corneal changes and thinning.
