The Parathyroid Glands00:59

The Parathyroid Glands

The two pairs of parathyroid glands embedded within the posterior surface of the thyroid gland are restricted by a dense capsule around them. These glands comprise two distinct cell populations—parathyroid oxyphil and parathyroid principal cells- pivotal in calcium homeostasis.
Oxyphil cells, whose functions remain elusive, emerge during late puberty, adding a layer of complexity to the parathyroid gland's intricacies. In contrast, principal parathyroid cells undertake a vital role by producing...

Mitral Stenosis II: Clinical features and Diagnostic Tests

Mitral stenosis is a heart condition in which the mitral valve, which allows blood to flow from the left atrium to the left ventricle, becomes narrowed or stenotic. This narrowing hinders blood flow and leads to clinical symptoms requiring specific medical evaluations and management strategies. The following overview outlines the clinical symptoms, assessments, diagnostic findings, prevention methods, and treatments for mitral stenosis.Clinical ManifestationsDyspnea (shortness of breath): This...

Imaging Studies for Cardiovascular System VI: Calcium -Scoring CT

Calcium-Scoring CT ScanA calcium-scoring CT scan, also known as coronary artery calcium (CAC) scan, detects calcium deposits in the coronary arteries. This test assesses the risk of coronary artery disease (CAD), which can lead to cardiovascular events such as angina, heart failure, and sudden cardiac arrest.A calcium-scoring CT scan is generally recommended for individuals at intermediate risk of CAD without symptoms. It includes:Men aged 40-75 and women aged 50-75: Especially those with a...

Mitral Regurgitation II: Clinical Features and Diagnostic Tests

Mitral regurgitation (MR) is a valvular heart disorder in which the mitral valve fails to close tightly, allowing blood to leak backward into the heart. Understanding the clinical manifestations, assessment, diagnostic findings, and medical management of MR is crucial to effectively managing affected patients.Clinical Manifestations of Mitral RegurgitationMitral regurgitation can be acute or chronic, each presenting differently and requiring different approaches:1. Acute Mitral...

Imaging Studies for Cardiovascular System V: CT

Cardiac computed tomography (CT) scanning is an advanced cardiac imaging technique that utilizes CT technology, with or without intravenous (IV) contrast, to produce accurate cross-sectional virtual slices of specific areas of the heart, coronary circulation, and major blood vessels such as the aorta, pulmonary veins, and arteries. The computer processes these slices to generate three-dimensional images. Multidetector CT (MDCT) is a rapid form of CT scanning that captures multiple slices...
Hormones and Bone Tissue01:17

Hormones and Bone Tissue

The endocrine system produces and secretes hormones, which interact with the skeletal system. These hormones control bone growth, maintain bone once it is formed, and remodel it.
Hormones That Influence Osteoblasts and/or Maintain the Matrix
Several hormones are necessary for controlling bone growth and maintaining the bone matrix. The pituitary gland secretes growth hormone (GH), which, as its name implies, controls bone growth. This happens in several ways: first, it triggers chondrocyte...

Summary

Three-phase contrast-enhanced CT accurately locates hyperfunctioning parathyroid glands. Density measurements differentiate adenomas from normal tissue and lymph nodes based on contrast enhancement patterns.

Area of Science:

  • Radiology
  • Endocrinology
  • Medical Imaging

Background:

  • Accurate localization of hyperfunctioning parathyroid glands is crucial for effective treatment of hyperparathyroidism.
  • Distinguishing parathyroid adenomas from normal thyroid tissue and lymph nodes can be challenging with conventional imaging techniques.

Observation:

  • Three-phase contrast-enhanced computed tomography (CT) demonstrates high sensitivity for parathyroid gland localization.
  • Spontaneous attenuation of parathyroid adenomas is typically below 80 Hounsfield Units (HU), while normal thyroid tissue density exceeds 80 HU.
  • Post-contrast imaging reveals distinct density patterns: adenomas exceed 130 HU at 45 seconds, whereas lymph nodes remain below 130 HU.

Findings:

  • Parathyroid adenomas exhibit a characteristic attenuation pattern after contrast injection, decreasing by over 20 HU between 45 and 70 seconds.

  • In contrast, lymph nodes show increasing attenuation within the same time frame (45-70 seconds).
  • Density measurements are valuable for differentiating parathyroid adenomas from adjacent structures like lymph nodes and thyroid tissue.
  • Implications:

    • This CT imaging technique enhances diagnostic accuracy for hyperfunctioning parathyroid glands, potentially improving surgical planning and patient outcomes.
    • The described density criteria can aid radiologists in differentiating parathyroid adenomas from benign lymphadenopathy and normal thyroid tissue.
    • Further validation of these findings could refine imaging protocols for diagnosing and managing primary hyperparathyroidism.
