A rare malignant peripheral nerve sheath tumor (MPNST) with unusual lipoblastic differentiation caused hydrocephalus and death in a patient. This case highlights a novel tumor subtype with potential diagnostic and therapeutic implications.

Background:
- Malignant peripheral nerve sheath tumors (MPNSTs) are rare, aggressive neoplasms arising from peripheral nerves.

- Microscopic analysis showed features of MPNST with rhabdomyoblastic and atypical lipoblastic differentiation.

- The tumor exhibited classical MPNST morphology with rhabdomyoblastic differentiation.
- Unique lobules of atypical adipose tissue (lipoblastic differentiation) were observed within the MPNST.
- This represents a previously unreported combination of differentiation in MPNST.
Implications:
- This case expands the known morphological spectrum of MPNST.
- Recognition of lipoblastic differentiation is crucial for accurate diagnosis and potential therapeutic strategies.
- Further research is needed to understand the behavior and origin of this rare MPNST variant.
