Background:

Observation:

  • A 54-year-old man with unstable angina underwent bare-metal stent implantation.
  • Eleven months post-implantation, he developed acute myocardial infarction following strenuous exercise.
  • Coronary angiography confirmed late stent thrombosis in the right coronary artery.

Findings:

  • Successful balloon angioplasty restored blood flow (TIMI-3) in the occluded stent.
  • The patient recovered and was discharged on dual antiplatelet therapy.
  • Mechanisms, implications, and preventive strategies for late stent thrombosis are discussed.

Implications:

  • Late stent thrombosis, even in bare-metal stents, poses a significant risk.
  • Heavy exercise may be a trigger for late stent thrombosis in susceptible individuals.
  • Further research into preventive therapies for late stent thrombosis is warranted.
