Background:

  • Pseudoxanthoma elasticum (PXE) is a rare genetic disorder affecting elastic tissues.
  • Perforating pseudoxanthoma elasticum (PPXE) is a variant characterized by transepidermal elimination of abnormal elastic fibers.

Observation:

  • A 58-year-old obese woman presented with a 3-year history of pruritic, yellowish plaques around the umbilicus and flat yellow papules on the axilla and neck.
  • A burn injury to the abdominal plaque resulted in the development of perforating papules and a violaceous hue.

Findings:

  • Histological examination revealed pathological elastic fibers with keratotic plugs and dermal perforation.
  • The findings were consistent with perforating pseudoxanthoma elasticum.

Implications:

  • This case suggests that mechanical factors, such as thermal injury, can induce or exacerbate perforation in PPXE.
  • Understanding these triggers is crucial for managing patients with this rare hereditary condition and preventing complications.
