Summary

Biermer's disease, linked to vitamin B12 deficiency, can increase stroke risk due to high homocysteine levels. Supplementing vitamin B12 may reduce this risk by lowering homocysteine.

Background:

  • Biermer's disease is an autoimmune condition causing vitamin B12 deficiency.
  • Ischemic stroke is an infrequent but serious complication associated with Biermer's disease.

Observation:

  • A 58-year-old male experienced recurrent ischemic strokes.
  • Investigations revealed elevated plasma homocysteine levels and pernicious anemia, with the patient being otherwise asymptomatic.

Findings:

  • Hyperhomocysteinemia, a condition of high homocysteine levels, is a significant risk factor for stroke.
  • Vitamin B12 deficiency, as seen in Biermer's disease, contributes to hyperhomocysteinemia.

Implications:

  • Vitamin B12 supplementation can lower homocysteine concentrations.
