The Parathyroid Glands00:59

The Parathyroid Glands

The two pairs of parathyroid glands embedded within the posterior surface of the thyroid gland are restricted by a dense capsule around them. These glands comprise two distinct cell populations—parathyroid oxyphil and parathyroid principal cells- pivotal in calcium homeostasis.
Oxyphil cells, whose functions remain elusive, emerge during late puberty, adding a layer of complexity to the parathyroid gland's intricacies. In contrast, principal parathyroid cells undertake a vital role by producing...

Inadvertent parathyroidectomy (IP) during thyroid surgery occurs in 7.9% of cases, increasing hypocalcemia risk. Malignant disease and lymph node dissection are key risk factors for IP and permanent hypocalcemia.

Area of Science:

  • Endocrine Surgery
  • Surgical Pathology
  • Thyroid Oncology

Background:

  • Inadvertent parathyroidectomy (IP) is a complication of thyroid surgery.
  • It can lead to temporary or permanent hypocalcemia.
  • Identifying risk factors is crucial for prevention.

Purpose of the Study:

  • To identify risk factors for inadvertent parathyroidectomy (IP) during thyroid surgery.
  • To evaluate the impact of IP on temporary and permanent hypocalcemia.
  • To reduce the incidence of IP and its complications.

Main Methods:

  • Retrospective review of 882 thyroidectomy procedures (January-December 2005).
  • Data collected included demographics, diagnosis, operative reports, pathology, and postoperative calcium levels.

  • Patients were grouped by presence or absence of IP and compared.
  • Main Results:

    • Inadvertent parathyroidectomy (IP) occurred in 7.9% of patients.
    • Risk factors for IP included younger age, malignant disease, total thyroidectomy with lymph node dissection, low specimen weight, and non-visualization of parathyroid glands.
    • Malignant disease and lymph node dissection were significantly correlated with IP and permanent hypocalcemia, respectively.

    Conclusions:

    • Inadvertent parathyroidectomy (IP) is a significant complication of thyroidectomy.
    • Malignant disease, lymph node dissection, and non-visualization of parathyroid glands are key risk factors.
    • Preventive strategies, such as avoiding parathyroid fragmentation, are necessary to reduce IP incidence and subsequent hypocalcemia.
