Background:

The cariostatic efficacy of different fluoride forms, including fluorapatite, calcium fluoride, and soluble/insoluble fluoride, remains a subject of debate.
Understanding the specific roles of various fluoride fractions is crucial for optimizing caries prevention strategies.

Purpose of the Study:

To investigate the relative cariostatic effects of different fluoride forms using an intraoral caries model.
To differentiate the impact of KOH-soluble and KOH-insoluble fluoride on enamel demineralization.

Main Methods:

Premolar enamel slabs were divided into control (untreated), KOH-insoluble fluoride, and KOH-soluble and insoluble fluoride groups after 2% sodium fluoride (NaF) treatment.
Slabs were subjected to an intraoral caries model over 4-week periods in human participants.
Quantitative microradiography was used to analyze mineral loss (delta Z) and lesion depth.

Main Results:

Control teeth exhibited significant mineral loss (1680 vol% μm) and lesion depth (90 μm).
KOH-insoluble fluoride treatment resulted in higher mineral loss (2167 vol% μm) and lesion depth (88 μm) compared to controls.
KOH-soluble and insoluble fluoride treatment significantly reduced mineral loss (620 vol% μm) and lesion depth (35.3 μm).

Conclusions:

Only the KOH-soluble fraction of fluoride demonstrated a significant cariostatic effect, reducing both mineral loss and lesion depth.
These findings suggest that the bioavailability and form of fluoride are critical determinants of its caries-preventive efficacy.
