Summary

Sevoflurane anesthesia can cause atrioventricular junctional rhythm (AVJR), a type of heart rhythm disturbance. This condition, known as isorhythmic dissociation, may lead to hypotension during surgery.

  • Atrioventricular junctional rhythm (AVJR) is a potential complication during anesthesia.

Observation:

  • A case of AVJR during sevoflurane maintenance anesthesia in a 61-year-old woman undergoing knee arthroplasty.
  • Electrocardiogram showed dissociated P-waves approaching, entering, and exiting QRS complexes, with transient ST depression.

Findings:

  • The phenomenon was diagnosed as AVJR with isorhythmic dissociation.
  • Discontinuation of sevoflurane resolved the isorhythmic dissociation, restoring sinus rhythm.

Implications:

  • Sevoflurane can induce atrioventricular conduction disturbances leading to isorhythmic dissociation and AVJR.
  • Clinicians should be aware of the potential for hypotension associated with isorhythmic dissociation during sevoflurane anesthesia.
