Surgical treatment for peptic ulcer disease (PUD) is declining, with emergency surgeries increasing relative to elective ones. Advances in medications may eventually make PUD surgery obsolete.
Area of Science:
- Gastroenterology
- Surgical Gastroenterology
- Epidemiology
Background:
- Peptic ulcer disease (PUD) management has shifted due to potent gastric acid suppressors and effective Helicobacter pylori antibiotics.
- The rate of surgical interventions for PUD and its complications has significantly decreased.
Purpose of the Study:
- To audit the experience with peptic ulcer disease and its surgical treatment at a tertiary health institution in Nigeria.
- To analyze trends in surgical PUD management over a five-year period.
Main Methods:
- A retrospective audit of hospital admissions for PUD from January 2000 to December 2004.
- Detailed analysis of case files for patients who underwent surgical treatment, including demographics, clinical data, and outcomes.
- Focus on Usmanu Danfodiyo University Teaching Hospital (UDUTH), Sokoto.
Main Results:
- Out of 624 PUD admissions, only 21 (3%) required surgical treatment.
- The majority of surgical patients (18/21) were male, with ages ranging from 30 to 62 years.
- Emergency procedures constituted 62% of surgeries due to life-threatening complications, with a mortality rate of 4.8%.
Conclusions:
- Elective peptic ulcer surgery has declined, while emergency procedures remain significant.
- The study suggests that medical advancements may eventually render surgical treatment for PUD historical.
