Summary

Reverse shoulder arthroplasty (RSA) can help patients with failed rotator cuff surgery. However, outcomes are worse than primary RSA, especially if patients have good preoperative arm elevation, risking decreased function and satisfaction.

Main Results:

  • Overall complication rate was 12% (5 patients), with 5% (2 patients) requiring re-operation.
  • Pseudoparalytic shoulders showed improved AAE (56 to 123 degrees) with 7% dissatisfaction.
  • Painful shoulders with AAE >= 90 degrees experienced decreased AAE (146 to 122 degrees) and 27% dissatisfaction.

Conclusions:

  • RSA can restore function in cuff-deficient shoulders post-rotator cuff surgery failure.
  • RSA outcomes are inferior compared to primary RSA.
  • Preoperative AAE >= 90 degrees in RSA candidates may lead to functional loss and reduced patient satisfaction.
