Background:

Renin-angiotensin system (RAS) blockade is crucial for managing patients with high cardiovascular risk.
Ramipril is a widely accepted 'reference' drug for RAS blockade.

Purpose of the Study:

To compare the cardiovascular protection of telmisartan versus ramipril in high-risk patients.
To evaluate the overall efficacy and tolerability of telmisartan compared to ramipril.

Main Methods:

The ONgoing Telmisartan Alone and in combination with Ramipril Global Endpoint Trial (ONTARGET) involved 23,400 high-risk cardiovascular patients.
Direct comparison of telmisartan and ramipril effectiveness and tolerability.

Main Results:

Telmisartan and ramipril were found to be therapeutically equivalent in providing cardiovascular protection.
