Brucellosis can cause myelofibrosis, a rare bone marrow disorder. This case report highlights successful treatment with doxycycline and rifampin, leading to complete recovery and resolution of myelofibrosis.

Area of Science:

  • Hematology
  • Infectious Diseases
  • Pathology

Background:

  • Brucellosis is a zoonotic infection with diverse clinical manifestations.
  • Pancytopenia and myelofibrosis are serious hematological conditions.
  • The association between brucellosis and myelofibrosis is not well-established.

Observation:

  • A 29-year-old woman presented with pancytopenia and myelofibrosis.
  • Brucella melitensis was identified as the causative agent.
  • The patient exhibited symptoms consistent with severe bone marrow involvement.

Findings:

  • The patient achieved complete recovery following treatment with doxycycline and rifampin.
  • Repeat bone marrow biopsy revealed hypercellularity, with resolution of myelofibrosis.

  • This case represents the first documented instance of brucellosis-induced myelofibrosis.
  • Implications:

    • Brucellosis should be considered in the differential diagnosis of myelofibrosis, particularly in endemic regions.
    • Early diagnosis and appropriate antibiotic therapy are crucial for favorable outcomes.
    • Further research is warranted to understand the pathogenesis of brucellosis-related myelofibrosis.
