Background:

  • Squamous atypia is a benign condition observed after tracheostomy.
  • It is often misdiagnosed as squamous cell carcinoma due to cellular morphology.

Observation:

  • A case of extreme squamous atypia is presented in a patient 30 years post-laryngectomy.
  • The atypia was severe enough to warrant extensive investigation for occult malignancy.

Findings:

  • Segmental bronchoscopy and endobronchial brushings revealed numerous atypical cells in the segmental bronchi.
  • This indicates that post-tracheostomy squamous atypia can extend beyond the tracheal stump to affect distant respiratory mucosa.

Implications:

  • Healthcare providers should be aware of this distant manifestation of post-tracheostomy atypia.
