Nursing Assessment of the Genitourinary System I: Health History01:21

Nursing Assessment of the Genitourinary System I: Health History

The genitourinary system is critical to maintaining fluid balance, waste elimination, and reproductive function. Nurses play a vital role in assessing this system, beginning with a thorough health history. This process involves gathering patient information, identifying risk factors, and recognizing symptoms of genitourinary disorders. Early detection is vital for timely interventions and management.1. Gathering Patient InformationA complete health history includes the patient’s personal,...
Hemodialysis I: Introduction01:25

Hemodialysis I: Introduction

Hemodialysis (HD) is a medical treatment that artificially removes waste products, excess fluids, and toxins from the blood when the kidneys are no longer able to perform these functions effectively. In this process, blood is filtered through a semipermeable membrane, allowing for the selective removal of waste while preserving necessary components like blood cells and proteins. Hemodialysis is typically performed in patients with end-stage renal disease (ESRD) or severe kidney...

Kidney Transplant II: Surgical Procedure

Preoperative ManagementThe primary goals of preoperative management in kidney transplantation are to optimize the patient’s metabolic state and prepare them for surgery through diet adjustments, necessary dialysis, and tailored medical treatment. This phase also involves comprehensive infection screening and patient education about the surgical procedure and postoperative care to improve outcomes and adherence.Medical ManagementA comprehensive evaluation is required for both the living donor...

Urinary Tract Calculi VI: Surgical Management

Procedures for Kidney StonesMedical intervention is necessary when kidney stones or renal calculi are too large to pass spontaneously (typically greater than 5 millimeters) when stones are accompanied by symptomatic infection (such as fever or pyelonephritis), when they impair kidney function, or when they cause persistent symptoms like severe pain, nausea, or urinary retention. Additionally, patients with only one kidney or those who cannot be treated with medical management also require...

Nursing Assessment of the Genitourinary System II: Inspection and Palpation

The nursing assessment of the genitourinary (GU) system involves a systematic inspection and palpation to identify abnormalities in the kidneys, bladder, and surrounding structures.InspectionMouth: Inspect for signs of kidney dysfunction, such as stomatitis (inflammation of the mouth) and ammonia breath, which may occur in advanced kidney disease due to the buildup of urea, breaking down into ammonia.Skin: Check for pallor, which could indicate anemia caused by kidney disease. Look for...

Anatomy of the Genitourinary System II: Bladder and Urethra

The lower urinary system consists of the urinary bladder and urethra, which are essential in storing and expelling urine from the body. Together with the internal and external sphincters, these structures work together to regulate urination effectively.Anatomy of the BladderThe urinary bladder is a muscular, stretchable organ behind the pubic bone and in front of the rectum. In females, the bladder is positioned anterior to the vagina and inferior to the uterus, while in males, it is located...

Summary

Genitourinary fistulae, often caused by obstructed labor, significantly impact quality of life. Surgical repair offers a definitive cure for these socially debilitating conditions.

Area of Science:

  • Urology
  • Gynecology
  • Public Health

Background:

  • Genitourinary fistulae present a significant challenge in certain populations.
  • Obstructed labor is a primary etiological factor, leading to socially debilitating conditions.

Purpose of the Study:

  • To analyze the incidence, etiopathology, diagnosis, and therapeutic outcomes of genitourinary fistulae.
  • To evaluate surgical and conservative management strategies in an Indian population.

Main Methods:

  • Retrospective analysis of 558 genitourinary fistula cases treated between 1990-2006.
  • Surgical approaches included transvaginal and transabdominal routes, with grafts/flaps used as needed.
  • Outcomes assessed included incidence, etiology, surgical techniques, and success rates.

Main Results:

  • Vesicovaginal fistulae were most common (403 cases), followed by ureterovaginal (84), urethrovaginal (43), and vesicouterine (28).
  • Obstructed labor (72.2%) and hysterectomy were leading causes.
  • Surgical repair demonstrated excellent results, with conservative management successful in select cases.

Conclusions:

  • Genitourinary fistulae, while not life-threatening, cause severe social distress.
  • Surgical intervention is the definitive treatment, though conservative approaches have a role in specific instances.
