The study found that functional integration in large medical groups is strongly associated with chronic care model systems. Structural and financial integration showed weaker links to these essential chronic disease management tools.
Area of Science:
- Healthcare Management
- Health Services Research
- Chronic Disease Management
Background:
- Effective chronic care management requires integrated systems within medical groups.
- The Chronic Care Model (CCM) provides a framework for improving chronic illness care.
- Understanding the relationship between organizational integration and CCM implementation is crucial for healthcare delivery.
Purpose of the Study:
- To examine the association between recommended Chronic Care Model (CCM) systems and the level of integration in large medical groups.
- To determine if structural, financial, or functional integration is more closely linked to the presence of CCM components.
Main Methods:
- A cross-sectional survey was conducted in 2007 with medical directors of large national medical groups (≥100 physicians).
- The survey assessed integration (structural, financial, functional) and the presence/use of CCM practice systems using the Physician Practice Connections-Readiness Survey (PPC-RS).
- Statistical analysis tested the relationship between integration measures and PPC-RS scores, controlling for group attributes.
Main Results:
- A strong correlation was observed between the total PPC-RS score and functional integration (r = 0.53, P <.01).
- Delivery system redesign (r = 0.27-0.52) and decision support (r = 0.21-0.46) showed the strongest correlations with PPC-RS components.
- Organizational characteristics had minimal impact on these observed relationships.
Conclusions:
- Functional integration within medical groups is significantly related to the adoption of CCM practice systems.
- The potential for integration (structure, finance) is less strongly associated with CCM components than demonstrated functional integration.
- These findings highlight the importance of functional integration for successful implementation of chronic disease management strategies.
