A novel muscle-sparing approach to the thoracoabdominal aorta via the 3rd intercostal space offers improved surgical access for thoracoabdominal aortic aneurysm repair, with no hospital deaths in 16 patients.
Area of Science:
- Cardiovascular Surgery
- Thoracic Surgery
- Vascular Surgery
Background:
- Thoracoabdominal aortic aneurysms (TAAA) pose significant surgical challenges.
- Standard surgical approaches may involve extensive muscle dissection and associated morbidity.
- Improved access is crucial for effective TAAA repair.
Purpose of the Study:
- To describe a modified muscle-sparing high approach for TAAA repair.
- To evaluate the safety and efficacy of this novel surgical technique.
Main Methods:
- A modified muscle-sparing approach through the 3rd intercostal space was utilized.
- Sixteen patients with Type I and II TAAAs underwent aortic graft replacement using this technique.
- Data on surgical outcomes, including mortality and postoperative pain, were collected.
Main Results:
- No hospital deaths occurred among the 16 patients.
- The approach provided improved surgical access for TAAA repair.
- Three patients (18.8%) experienced severe postoperative pain requiring prolonged analgesia.
Conclusions:
- The modified muscle-sparing high approach is a safe and effective alternative for TAAA repair.
- This technique offers advantages over the standard approach via the 6th intercostal space.
- Further research may explore long-term outcomes and pain management strategies.
