Background:

Autoimmune thyroid diseases (AITDs) often co-occur with other endocrine autoimmune conditions, forming polyglandular autoimmunity (PGA).
The cytotoxic T-lymphocyte antigen 4 (CTLA-4) gene, a T-cell inhibitor, and tumor necrosis factor alpha (TNF-alpha), a proinflammatory cytokine, have been implicated in AITD and PGA.
Genetic variations in CTLA-4 and TNF-alpha may influence susceptibility to PGA.

Purpose of the Study:

To investigate the association between CTLA-4 CT60 and TNF-alpha-863 gene polymorphisms and PGA.
To determine if specific genotypes or alleles of CTLA-4 and TNF-alpha are more prevalent in patients with PGA compared to healthy individuals or those with single autoimmune conditions.

Main Methods:

Genotyping of CTLA-4 CT60 and TNF-alpha-863 polymorphisms using minisequencing.
Analysis of homogeneous groups: 70 AITD patients, 70 type 1 diabetes (T1D) patients, 70 PGA patients (AITD + T1D), and 100 healthy controls.
Statistical analysis including odds ratios (OR) and confidence intervals (CI) to assess associations.

Main Results:

The CTLA-4 CT60 G/G genotype was significantly more frequent in PGA patients (48.6%) than in controls (32.0%), with an OR of 2.01 (p=0.038).
The predisposing G allele of CTLA-4 CT60 was also increased in PGA patients (OR=1.63, p=0.042).
No significant associations were found for TNF-alpha-863 with PGA, although a trend was observed for T1D patients (OR=1.81, p=0.079).

Conclusions:

The CTLA-4 CT60 polymorphism is significantly associated with polyglandular autoimmunity.
This finding suggests a genetic link between CTLA-4 variations and the development of multiple endocrine autoimmune diseases.
TNF-alpha-863 polymorphism did not show a significant association with PGA in this study population.
