Serious bacterial keratitis in hospitalized patients is often linked to contact lens wear. However, contact lens wear was not a negative prognostic factor in this study.

Purpose:

  • To investigate the epidemiologic and microbiologic features of hospitalized bacterial keratitis cases.
  • To identify and define the risk factors associated with severe bacterial keratitis.
  • To evaluate the prognostic impact of various factors on visual outcomes.

Summary:

  • A retrospective study of 67 hospitalized bacterial keratitis patients (Jan 2006-Nov 2007) revealed mean age of 46.
  • Contact lens wear (49%) was the leading local risk factor, followed by keratopathy (16%) and corneal injury (12%).
  • Gram-negative bacteria were common (45%), especially in contact lens wearers. 85% of patients achieved improved visual acuity after 3 months.

Impact:

  • Contact lens wear, while a primary risk factor, did not negatively impact prognosis.
  • Low preoperative visual acuity, advanced age, and large initial infiltrate size are poor prognostic indicators.
  • Immunodeficiency, particularly due to diabetes and Gougerot-Sjögren's syndrome, is associated with the worst visual outcomes.
