Summary

This study reports the first observed instance of clear cells within a neurofibroma. Researchers suggest these clear cells may result from degenerative changes in the skin lesion.

Background:

  • Clear cell changes are documented in various cutaneous lesions, including nevi and carcinomas.
  • However, clear cells have not previously been reported in neurofibromas.

Observation:

  • A biopsy of a clavicle papule revealed clear cells positive for S-100 and CD68, initially suggesting a clear cell granular cell tumor.
  • Excision confirmed the clear cell proliferation and identified an adjacent neurofibroma with transitional areas.

Findings:

  • This case represents the first documentation of clear cells occurring within a neurofibroma.
  • Immunohistochemical staining (p75) confirmed the presence of both cell populations.

Implications:

  • The findings suggest that clear cells in this context may represent a degenerative phenomenon within neurofibromas.
  • This expands the differential diagnosis for cutaneous clear cell proliferations.
