Summary

Infrahepatic vena cavocavostomy offers a primary anastomosis option for liver transplants, avoiding the need for venovenous bypass. This technique, used in two cases, shows promise as an alternative for selected patients.

Area of Science:

  • Abdominal surgery
  • Transplant surgery
  • Vascular anastomosis

Background:

  • Standard liver transplant venous anastomosis (piggyback) can develop stenosis, requiring rescue therapy.
  • Infrahepatic vena cavocavostomy is typically a rescue procedure for obstructed venous outflow from liver allografts.

Observation:

  • This study describes two adult liver transplantations where infrahepatic vena cavocavostomy was utilized as the primary venous anastomosis.
  • A caval clamp above the retrohepatic portion caused partial caval flow obstruction, which was well tolerated.

Findings:

  • The primary use of infrahepatic vena cavocavostomy in these cases successfully established venous outflow.
  • Hemodynamic obstruction during the procedure was tolerated, negating the need for venovenous bypass.

Implications:

  • Infrahepatic vena cavocavostomy may be a viable primary anastomosis alternative in select liver transplant recipients.
  • Further research is warranted to establish the broader applicability and long-term outcomes of this technique.
