Summary

A rare case of ectopic thymic adenoma causing hypercalcemia in a child was diagnosed after initial imaging was inconclusive. Surgical removal successfully resolved the persistent hypercalcemia and bone lesions.

Observation:

  • A 10-year-old girl presented with fatigue, hypercalcemia, and osteolytic bone lesions.
  • Initial imaging suggested a thyroidal or parathyroidal mass, but exploration revealed thymic tissue without an adenoma.
  • Despite extensive imaging, the ectopic adenoma was only located by MIBI-SPECT/CT near the left jugulum after two years of worsening symptoms.

Findings:

  • Histological examination confirmed thymic tissue, not a typical adenoma.
  • Persistent hypercalcemia and decreasing bone mineral density indicated an ongoing issue.
  • MIBI-SPECT/CT successfully localized the ectopic thymic adenoma, which was surgically removed.

Implications:

  • This case highlights the diagnostic challenges of ectopic thymic adenomas, especially in pediatric hypercalcemia.
  • Advanced nuclear imaging techniques like MIBI-SPECT/CT are crucial for locating elusive ectopic endocrine tissue.
  • Timely diagnosis and surgical excision of ectopic thymic adenomas can effectively manage hypercalcemia and prevent bone loss.
