The Mammary Glands01:12

The Mammary Glands

The female breast is a hemispheric projection of variable size positioned anterior to the pectoralis major and serratus anterior muscles. A fascia layer composed of dense, irregular connective tissue connects it to these muscles.
Each breast features a pigmented projection known as the nipple, through which milk emerges via closely spaced openings of ducts, referred to as lactiferous ducts. Surrounding the nipple is a circular pigmented area of skin named the areola, which appears rough due to...

Area of Science:

  • Plastic Surgery
  • Surgical Techniques
  • Aesthetic Medicine

Background:

  • Accurate nipple-areola complex (NAC) localization is crucial in breast reduction surgery.
  • Traditional methods for NAC placement can be subjective and challenging, especially with large breasts.
  • Difficulty in precise NAC localization can lead to secondary correction issues.

Purpose of the Study:

  • To introduce a simple and accurate method for nipple localization in breast reduction.
  • To decrease the learning curve associated with NAC placement for trainee surgeons.
  • To improve the consistency and aesthetic outcome of nipple placement.

Main Methods:

  • A geometric approach using an equilateral or short isosceles triangle is described.
  • The triangle's apex is at the sternal angle, with lower angles marking the new NAC position.
  • Measurements of the triangle's base and vertical limb allow for calculation and marking of the NAC site.

Main Results:

  • The described technique was successfully applied in 40 patients.
  • Satisfactory placement of the nipple-areola complex was achieved in all cases.
  • The method demonstrated effective nipple localization for breast reduction.

Conclusions:

  • This technique offers objective measurements for nipple localization in breast reduction.
  • The method is easy to learn and practice, enhancing trainee confidence.
  • It provides a reliable approach for aesthetic nipple placement in breast reduction surgery.
