Objectives:
To describe the pharmacokinetics of the different antiepileptic drugs. To discuss optimal pharmacologic use of antiepileptic therapy as it relates to elderly individuals taking multiple medications. To understand the controversy over generic versus brand-name antiepileptic drugs.

Conclusions:
While the goal of antiepileptic drug therapy is to have patients be seizure-free without experiencing side effects, this ideal may be difficult to achieve. Classical and newer antiepileptic drugs have varying mechanisms of action and pharmacokinetics that consultant pharmacists must understand. Because of the potential of drug interactions, selecting an appropriate antiepileptic drug requires a thorough assessment of patient health history to determine which therapy will be most effective and safe. Generic options are available and are effective for some, but concerns about bioequivalence make their use controversial.
