Insights
Ventilator-associated pneumonia (VAP) is a significant concern in South African pediatric intensive care units. This guideline provides evidence-based recommendations for VAP diagnosis, prevention, and antimicrobial use in children.
Area of Science:
- Critical Care Medicine
- Pediatric Infectious Diseases
- Public Health
Background:
- Ventilator-associated pneumonia (VAP) is understudied in South Africa.
- VAP contributes to increased morbidity and mortality in pediatric intensive care units (PICUs).
Purpose of the Study:
- To review existing evidence and formulate recommendations for VAP prevention and management in children.
- To provide clear guidance for VAP care, limiting unnecessary therapies and inappropriate antimicrobial use.
Main Methods:
- A dedicated Working Group was established.
- A comprehensive literature review was conducted on pediatric VAP etiology, prevention, and management.
Main Results:
- Evidence-based clinical practice guidelines for VAP diagnosis and prevention in South Africa have been developed.
- The current landscape of antimicrobial use for VAP has been critically reviewed.
Conclusions:
- Clear recommendations for VAP diagnosis and prevention are established for South African pediatric intensive care settings.
- Specific guidance on appropriate antimicrobial stewardship for VAP is provided.
Objective:
Ventilator-associated pneumonia (VAP) has been poorly studied in South Africa, but is likely to be a significant problem, with resulting increased morbidity and mortality in the paediatric intensive care unit population. This guideline is intended to review the evidence and recommendations for prevention and management of VAP in children and to provide, where possible, clear advice to aid the care of these children, to limit costly and unnecessary therapies and--importantly--limit inappropriate use of antimicrobial agents,
Evidence:
The Working Group was constituted. Literature on the aetiology, prevention and management of paediatric VAP is reviewed.
Recommendations:
Evidence-based clinical practice guidelines are provided for VAP diagnosis and prevention in South Africa. In addition, the current status of antimicrobial use has been reviewed and clear recommendations are set out.

The nurse must practice strict medical asepsis and adhere to infection control guidelines to minimize healthcare-associated infections.
Enhance airway patency
Position the patient correctly to facilitate drainage of the affected lung segments. Manual or mechanical percussion and vibration can also be employed.

For bacterial pneumonia, antibiotics serve as the cornerstone of therapy. Initial treatment often begins with empirical antibiotics, tailored to the anticipated causative organism and adjusted based on culture results. Key antibiotic choices include:
