Summary

Patients with coffee grounds emesis (CGE) may have serious underlying conditions beyond gastrointestinal bleeding. Stable patients with CGE and no hemoglobin drop require evaluation for non-GI diagnoses.

Background:

  • Acute upper gastrointestinal (UGI) bleeding presents with hematemesis, melena, or hematochezia.
  • Initial management is standardized regardless of nasogastric (NG) aspirate findings.
  • Early upper endoscopy is standard practice, though fewer high-risk lesions are identified in patients with coffee-ground aspirates compared to bloody aspirates.

Observation:

  • A case series of 6 hemodynamically stable patients presenting with coffee grounds emesis (CGE) and admitted for UGI bleeding was reviewed.
  • Nasogastric lavage revealed coffee grounds that cleared easily.
  • None of the patients required blood transfusions, and endoscopy revealed no significant UGI lesions in 3 of the 6 patients.

Findings:

  • Despite admission for UGI bleeding, significant associated diagnoses included acute myocardial infarction, urosepsis, small bowel obstruction, bilateral pulmonary emboli, and acute renal failure.
  • These critical non-GI conditions were potentially overlooked due to focus on the gastrointestinal bleed.

Implications:

  • Hemodynamically stable patients with coffee-ground NG aspirates and stable hemoglobin/hematocrit levels warrant comprehensive evaluation for non-GI conditions.
  • This approach is crucial even while managing the apparent GI bleed to ensure timely diagnosis and treatment of potentially life-threatening comorbidities.
