Summary

Ruthenium-106 brachytherapy offers a viable alternative to enucleation for thick uveal melanomas, demonstrating comparable survival rates and preserving vision in many cases. This eye-sparing treatment is effective for posterior uveal melanoma management.

Main Results:

  • Brachytherapy patients were younger with smaller tumors (9.3 mm vs. 12.2 mm).
  • Melanoma-related mortality at 5 and 10 years was similar between brachytherapy and enucleation groups (20.5% vs. 28.1% and 46.2% vs. 44.0%).
  • 71.4% of eyes treated with brachytherapy were preserved, with 70.8% achieving visual acuity of 20/200 or better.

Conclusions:

  • Ruthenium-106 brachytherapy is a safe and effective alternative to enucleation for select cases of thick posterior uveal melanoma.
  • This brachytherapy approach can preserve the eye and maintain useful vision.
  • Further research may refine patient selection for this eye-sparing modality.
