Summary

Lower urinary tract dysfunction (LUTD) is common in acute disseminated encephalomyelitis (ADEM), affecting one-third of patients. While associated with neurological deficits, LUTD does not predict overall functional outcome, though symptoms may persist.

Background:

  • Lower urinary tract dysfunction (LUTD) is well-documented in multiple sclerosis.
  • Its prevalence and outcomes in acute disseminated encephalomyelitis (ADEM) are less understood.

Purpose of the Study:

  • To investigate the occurrence and characteristics of LUTD in ADEM patients.
  • To correlate LUTD with other neurological deficits and functional outcomes.

Main Methods:

  • Evaluation of ADEM patients with significant LUTD using symptom analysis, ultrasonography, and urodynamics.
  • Management included antimuscarinics for storage symptoms and catheterization for voiding dysfunction.

Main Results:

  • 20 out of 61 (33%) ADEM patients presented with LUTD.
  • Voiding dysfunction, particularly urinary retention, was common. Detrusor overactivity/underactivity observed in urodynamics.
  • LUTD correlated with frontoparietal white matter changes and lower limb pyramidal involvement but not functional outcome.

Conclusions:

  • LUTD is a frequent complication of ADEM, often linked to neurological deficits.
  • While not impacting overall functional recovery, LUTD may persist, indicating incomplete resolution.
