Area of Science:

  • Neurology
  • Psychiatry

Background:

  • Psychiatric symptoms can arise from focal neurological deficits.
  • Cerebral infarctions, particularly in specific brain regions, may present with neuropsychiatric manifestations.

Observation:

  • A 79-year-old male with no prior psychiatric history presented with acute onset of auditory hallucinosis and persecutory delusions.
  • Neuroimaging revealed a small cerebral infarction in the left parietal region.

Findings:

  • The patient's psychotic symptoms were strongly associated with the identified cerebral infarction.
  • Psychiatric symptoms resolved following treatment with the antipsychotic medication perphenazine.

Implications:

  • This case highlights the potential for focal cerebral infarctions to trigger psychiatric symptoms in older adults.
  • Early diagnosis and targeted treatment, including antipsychotics and potentially stroke management, are crucial for managing such conditions.
